Ready for your child’s home away from home to be Pediatric Haven? Complete our enrollment paperwork for intake and we will collaborate with your child’s Pediatrician and insurance company to start the process.

Does My Child Qualify?
To qualify for admission, the following criteria must be met:
Be medically complex and/or technologically dependent
Have a physician’s order
Not be at risk of infection to other children or staff
Be complaint to physician’s plan of care
